Some pets require sedation for adequate physical exam, treatment and diagnostic tests. If sedation is necessary, I will assume responsibility for costs involved. The doctor(s) and staff of Stoneside Veterinary Hospital use all reasonable precautions against injury, escape, or death of my pet. The clinic and staff will not be held liable for any problems that develop provided reasonable care and precautions are followed. I understand that any problem(s) that develop with my pet if I cannot be contacted will be treated as deemed best by the veterinarian and I assume all responsibility for the treatment expenses involved. If I neglect to pick up my pet within 5 days of the date above and do not notify you within that time frame you may assume that the pet is abandoned and hereby authorized to dispose of the pet as you would deem best and/or necessary. I also understand that if fleas or ticks (or signs of these) are found on my pet, treatment will be administered at my cost.
